Community Nursing
Care

We provide community nursing supports to participants who require higher levels of care and assistance within the comfort of their homes or living setting.

Daily
Personal Activities

Our carers will assist and support participants to ensure that their personal care and hygiene are always upheld within the comforts of their home.

Shared
Living Arrangement

We provide assistance with, or supervision of, tasks of daily life in a shared living environment, with a focus on developing the skills of each individual to live as autonomously as possible.

Household
Tasks

We provide support to enable participants to maintain their home environment. This may involve essential household tasks that the participant is not able to undertake.

Transport
Assistance

Transport enables participants to access disability supports outside their home, and to achieve the goals in their plan.

Community
Participation

Our carers are ready to assist with or supervise a participant to engage in community, social, recreational, or economic activities.

About Us

We are four registered nurses who met and worked at the Vax Hub during the pandemic.

During our stint at the hub, we saw the public every day and realised that there is a number of disabled participants in the community who need help and assistance.

Our passion started from our unified advocacy that people with disabilities should be able to connect and socialise with their community without barriers and difficulties.
